Patient Forms

If you are a new patient to our office, you will receive a link to complete your New patient forms prior to your appointment time as well as anytime that updated forms are needed.  You will be able to upload a picture of your ID, and Insurance card. This will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

 
Dental Insurance

Please contact our office for Insurance details.  We look forward to hearing from you.

We are providers for the insurance providers listed above.  Some of our patients are 'out-of-network', which only means we do not participate with their insurance plan. Prior to any treatment, we will happily do a comprehensive benefits check for you. Our staff will gladly prepare all the necessary forms and submit a pre-treatment estimate to your insurance company to help you determine your benefits.  However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

Financing Option

We work with Care Credit to offer our patients the option of obtaining their own financing, with approved credit.This allows you to begin your treatment immediately – then pay for it over time with convenient monthly payments that help fit your budget.
